Our
geriatric care managers will meet with you in your home
to help evaluate your situation, listen to your goals and needs
and develop a plan of care for you. This information, specific
to your situation, will enable you to make well informed decisions.
Care Managers also:
- Assist
in the decision making process.
- Monitor
the level of care given to the older adult.
- Coordinate
household chores, meals, maintenance, bill paying and banking.
- Accompany
client to doctors’ appointments.
- Assist
in obtaining and coordinating home care, nursing home care,
and care in retirement or extended care facilities.
- Offer guidance
to out-of-town family members and act as liaison between the
family, older adult, physician and attorney and others.
- Advocate
for client and family’s needs.
- Reassess
and reevaluate the plan of care as needed and make appropriate
changes.
